Image attachments failed to upload but Radiant didn't report an error

Recently my Mastodon instance had a configuration issue that caused image uploads to fail.

Elk.zone caught it and reported the issue:

However, when I posted the same content with an attachment via Radiant, Radiant didn't catch the error and instead published the text of my post without the image. I didn't discover this until several hours and four image-less posts later.

I'm not sure if this is related to this post, but Radiant should provide better error handling and media attachment verification before publishing posts.
